Registration

UPDATE: Registration is now closed. 

There is a 15 person capacity for this event. Once we reach that number, registration will close. 

NAVWAR HQ requires all visitors to complete form DD3150. Completed forms can be sent to Michael Minneman. 

• Persons answering "I have not been vaccinated" or "I decline to respond" are required to have a negative test within 72 hours (rapid antigen, rapid PCR, regular PCR; proof of antibodies will not be accepted). A photo of a negative result within 72 hours will be accepted. 
• Persons answering "I have not been vaccinated" or "I decline to respond" are required to maintain 6' of separation within the meeting space.
