Form DD 3150
Recent guidance released by the Deputy Secretary of Defense requires that all travelers to the Naval Postgraduate School (NPS) attest to their vaccination status via DD Form 3150. Please note all attendees and speakers must carry a completed DD Form 3150 that must be kept on their person and shown at the symposium check-in. At this time, NPS is not accepting negative COVID test results in lieu of this document. All attendees must be fully vaccinated, no exceptions.

Additionally, NDIA will require all attendees to provide proof of full vaccination within three (3) days prior to badge pick-up. Details for securely providing vaccination documentation will be provided shortly.
