If you plan to attend Tuesday, January 7, please only select the committee meeting you will ACTUALLY be attending and not all boxes. Please be sure to share this information with the person who is registering you online so that it is completed properly.

CANCELLATION FEE: $15
You may cancel your registration and receive a partial refund, minus the $15 cancellation/processing fee by COB December 30, 2014. No cancellation fee will be required for the cancellation of the CF Pre-Meeting.
If cancellation occurs after the January 2 cut-off date, no refund will be offered. Substitutions are welcome and encouraged, please email Jennifer Sprinkel, CMP, CGMP at firstname.lastname@example.org with any cancellation/substitution requests.
